The Community Rehabilitation Worker will provide rehabilitation based interventions to support people to rebuild their lives following Brain Injury. Achieve outcomes and progression for service users with acquired brain injury in accordance with the allocated contract specification.

The Community Rehabilitation Worker will support the progression of service users with brain injury through a person centred approach to Rehabilitation, working for our base in Galashiels and throughout the Scottish Borders, providing outreach services in the community.

The Community Rehabilitation Worker will promote the Borders Brain Injury Service to potential referral organisations and individuals in order to attract starts onto the programme. The candidate must have experience in of working with people who have experienced Brain Injury, Neurological condition, Mental health issues or complex needs or similar cohort.

The Community Rehabilitation Worker will manage a caseload of Service Users to ensure they are supported to follow a course of Rehabilitative interventions as directed by initial assessment and action planning.

KEY RESPONSIBILITIES

• To fully participate in the recruitment, induction and initial assessment of Service Users. To deliver a person centred approach to rehabilitation in a one to one capacity
• To conduct an in-depth initial needs assessment using “Well-being Star”. Produce a series of actions to ensure each Service User had a tailored action plan to meet their individual needs.
• Conduct one to one sessions to explore the consequences of the Service Users brain injury and the effect on the individual’s functional abilities. Develop and implement specific strategies to assist with the process of rehabilitation.
• Plan, develop, implement and review appropriate interventions to promote community integration for individuals, their family members and carers.
• To provide brain injury awareness information for Service Users, their family members or carers.
• To conduct regular reviews of actions with Service Users

Performance Management and Delivery
• To take all opportunities to market the programme to appropriate organisations
• To develop relationships and networks with key stakeholders including NHS Borders and Lothian, and third sector and public sector organisations.
• To process documentation accurately and timeously to ensure appropriate information is collated for claim and statistical purposes

Quality and Continuous Improvement
• To maintain accurate participant records to ensure evidence of their rehabilitation journey.
• Report on caseloads and areas of responsibility as required. This will include producing reports on a monthly basis on areas such as starts, initial assessment, reviews, highlighted Service User achievements and producing good news stories.
• Maintain the highest standards of participant care at all times
• Complete all company documentation required as part of the role accurately and timeously
• To contribute to the wider staff team at monthly team meetings.
• To regularly gather feedback from Service Users on programme delivery and possible improvements.
• To actively pursue appropriate CPD opportunities.

General
• To attend training courses as identified and agreed in the Personal Development Plan
• To understand all aspects of the Company’s Equality Policies and Quality Management Systems and comply with and action, as appropriate, all of their regulations
• To understand personnel policy, including the Grievance and Disciplinary procedures of the company and ensure that these are rigidly adhered to at all times
• To ensure all Health and Safety procedures are adhered to, reporting any concerns/deficiencies to the nominated Health and Safety representative
• To ensure adherence with the policies and procedures related to the use of Web, Email and Mobile Phones
• To uphold and promote the good name and reputation of the company in all their dealings/contacts with participants. Involved/interested parties and the general public


PERSON SPECIFICATION

Education and training:
• Educated to HNC (SCQF Level 7) or equivalent is specified as desirable but relevant experience is of greater weight.
• A full driving license (E)

Work experience:
• Recent and relevant experience of working with people who have experienced ABI, TBI or Neurological condition.
• Experience of assessing an individual’s needs and translating then into realistic and achievable goals.
• Experience of leading 1:1 sessions and group
• Experience of developing, running and sustaining creative and stimulating activity based group sessions.
• Experience of recruiting, training, deploying and managing volunteers.
• Ability to influence and negotiate required outcomes and proactively promote services to a range of individuals
• Possess high standards and have excellent organisational skills, being able to prioritise workloads and deadlines

Skills and Knowledge:
• Ability to work confidentially ay all time both in and out of the work place.
• Ability to work without direct supervision
• Ability to priorities own workload
• Ability to work as part of a larger team and individually when required
• Ability to deliver presentations to groups internally and externally
• Excellent communications skills, listening, written and oral
• Flexible approach to working hours, duties and arrangements and able to work within agreed timescales.
• Knowledge of ABI, TBI and Neurological conditions and their effects on individuals and their family members.
• Should have strong administration skills, good computer/technology skills, basic numeracy/spreadsheet skills and good written communication/record keeping skills.
